If you are purchasing a puppy without CKC registration papers then you need to fill out this application. You can either print out this contract and sent via mail or bring it with you when you come to place deposit/pick up your puppy. OR you may send in by E-mail as a Word Document or Note Pad and sent to chicabonitachihuahuas@yahoo.com

In the subject line please put "Neuter Contract"

 **The following agreement is between ___________________________ the NEW owner and Michelle Mollett the original owner and breeder. Purchase price of $_________, of which Deposit amount of $__________ is non refundable for this male/female chihuahua puppy to Buyer as of the date specified below with the following conditions.

The Buyer agrees to spay/neuter the above puppy 6 months after purchase date. Buyer agrees to supply "original owner/breeder" with a veterinarian's letter by certified mail as proof of spay/neuter. If Buyer fails to spay/neuter puppy and/or provide proof by 6 months after purchase date all rights and ownership of the puppy will automatically terminate and revert back to "original owner/breeder" who may immediately resume possession of the puppy. 

Buyer agrees to stay in contact with the "original owner/breeder" of puppy until proper spay/neuter has been completed.       INITIALS___________
